Merit Woods Pharmacy, Inc.

19325 Mack Ave, Grosse Pointe Woods, MI 48236 | (313) 882-0922 | pharmacy@meritwoods.com

Effective date: November 2, 2026

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our duties

The law requires us to keep your health information private and to give you this notice of our legal duties and privacy practices. We must follow the notice currently in effect. We will tell you promptly if a breach occurs that may have compromised the privacy or security of your information. We can change this notice, and the changes will apply to all information we have about you. The new notice will be available on request, in our pharmacy and on this website.

How we use and share your information

- Treatment: We fill your prescriptions, counsel you, and talk with your doctors and other providers about your care. Example: we call your doctor about a drug interaction.

- Payment: We bill and get paid by your health plan or other payers. Example: we send your prescription information to your insurer to process a claim.

- Health care operations: We run our pharmacy and improve care. Example: quality reviews, training, and audits. We may also send refill reminders.

- Business associates: Companies that do work for us (for example, our pharmacy software vendor) may receive information, and they must protect it by contract.

Other ways we may share your information, as the law allows or requires

- When required by law, including reporting controlled-substance prescriptions to Michigan's prescription monitoring program (MAPS) and immunizations to the Michigan Care Improvement Registry (MCIR).

- Public health and safety: preventing disease, reporting adverse drug reactions and product problems to the FDA, product recalls, reporting suspected abuse, neglect, or domestic violence, and preventing a serious threat to health or safety.

- Health oversight agencies, such as the Michigan Board of Pharmacy and the Michigan Department of Licensing and Regulatory Affairs (LARA), for inspections, audits, and licensing.

- Lawsuits and legal actions, in response to a court or administrative order, subpoena, or similar process.

- Law enforcement, as allowed by law.

- Coroners, medical examiners, funeral directors, and organ or tissue donation organizations.

- Workers' compensation, special government functions such as military and national security, and research meeting legal requirements.

- People involved in your care: unless you tell us not to, we may share information with a family member or friend who picks up your prescriptions or helps pay for your care. If you can't tell us your preference, for example if you are unconscious, we may share it if we believe it is in your best interest.

Stricter state law

Some Michigan laws give certain information more protection, such as HIV and communicable disease information and mental health records. When a law is stricter than HIPAA, we follow it.

Substance use disorder records

If we receive records protected by federal substance use disorder confidentiality rules (42 CFR Part 2), we will not use or share them in any civil, criminal, administrative, or legislative proceeding against you without your written consent or a court order that meets Part 2 requirements.

Uses that need your written permission

We will not use or share your information for marketing or sell your information without your written authorization. We will not use or share psychotherapy notes without it either, though we do not normally keep them. Any other use not described in this notice also requires your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Your rights

- Get a copy: You may see or get a paper or electronic copy of your pharmacy records. We may charge a reasonable, cost-based fee.

- Ask us to correct your records if you think they are wrong or incomplete. We may say no, but we will tell you why in writing.

- Ask for confidential communications: You may ask us to contact you a certain way, such as a different phone number or address. We will agree to reasonable requests.

- Ask us to limit what we use or share. We do not have to agree, except one case: if you pay for an item or service in full out of pocket and ask us not to share that information with your health plan, we will say yes unless the law requires us to share it.

- Get a list of those we have shared your information with (an accounting) for up to six years before the date you ask, with certain exceptions. One list a year is free.

- Get a paper copy of this notice at any time, even if you agreed to receive it electronically.

- Choose someone to act for you: a person with your medical power of attorney or your legal guardian may exercise your rights.
